This is a scenario where the mother and baby die in childbirth?

I write a book, and the baby mother both died in childbirth. How can this happen in real life time? I know nothing about this type of thing.

Placenta previa where the placenta covers the opening to the cervix. If not found by ultrasound, the mother can bleed to death (thought placenta) and the baby dies from oxygen starvation. It is rare to see someone die from it these days (in the least developed countries), as is usually found in time and caesarean sections are performed.
